CKD is defined as kidney damage or glomerular filtration rate (GFR) < 60 ml/min/1.73 m2 for 3 months or more, irrespective of cause.

Women = (140–Age) × Weight [Kg] / (72 × SCr [mg/dL]) × 0.85

Ideal Body Weight (IBW), Men = 50 + [2.3 × (Height in inches – 60)]

Adjusted Body Weight (ABW) = IBW + 0.4 × (TBW – IBW)
